Community Home Care was developed to give you or your loved one the short-term care and assistance needed after a hospital stay, outpatient surgery or even a physician office visit. We understand the difficulties that come with managing such care, that figuring out what to do and how can be overwhelming, and we're here to help.

WHAT HOME CARE INCLUDES

Community Home Care provides skilled nursing, certified home health aide, which includes things like bathing and personal grooming, physical, and occupational therapy, social services, and medical supplies.

Our goal is to give you the support you need for your individual situation. And because we consider Community Home Care an extension of Helen Keller and Red Bay Hospital, you can be assured you'll always get the same quality care under the same high standards.

HOW IT WORKS

First we'll assess your health care needs with your doctor. Based on this, he or she will prescribe and manage the services necessary. Then we'll bring those services to your home based on your individual needs. A registered nurse will coordinate and closely monitor your care at all times and be a quick connection to your doctor should a situation arise that calls for immediate attention.

HOW PAYMENT IS HANDLED

Our business office will help you determine who pays what for you. Most services are covered by insurance, Medicare or Medicaid. In order for home care to be covered by any form of insurance, the patient must be home bound, under the care of a doctor and in need of skilled nursing, physical therapy or speech therapy on an intermittent basis.
